You’re missing the core of that point you make: Power Dynamics.

A Harem is a POWER. FANTASY. Full stop. So if the MC CAN screw another guy’s wife (usually excused by the husband’s mistreatment of said wife to make her want to look around – or actual physical abuse in some stories) it does NOT break the power fantasy. So the story can flow properly for what it’s intended to be. It’s far less jarring frankly. Because it feeds into the expected narrative.

NTR ANY other way, is about stripping the power away from the Player Avatar (which is often taken to be the MC – as it’s easiest done that way, rather than assuming the player will identify with a faceless narrator) and the larger problem with it in these style of games is a simple lack of PLAYER CHOICE. What to see; what not to see. How far it will go. Whether you want to see it at all – can it be fought back against, or is it on one side only (usually the case, though that implies that the wife/GF/LI has so much power over MC by a pussy she’s not even usually factually GIVING to him to start with, that he just can’t walk out the door and find a girl for himself. Who IS willing to give him the time of day)

A lot of men already go through the “caged in a relationship” bit in real life. Literally that’s what marriage is in the US and Canada until the family laws become a LOT more evened out again. She can leave; decide to fuck anyone else she wants, and even perhaps make you watch as her boyfriend cums inside while she’s ovulating… but unless you get that on tape (and frankly taping it without her consent is STILL illegal in most countries, so good luck getting empirical evidence) or can otherwise somehow prove HER the infidel in the relationship she is all but guaranteed alimony. Child support. So yeah… most of us want to feel empowered. Most men prefer to be dominant in sex. Or would, if they could find a woman who wanted that, and made her mind up that it was always allowed. Most of the audience for this style in game is probably straight. I won’t say they’re all white, but they’re more likely to be male at any rate.

So if it comes to a difference of Can I play like I have all the power, and EVERY woman wants me, regardless of whether it makes sense in reality; my answer is yes. I want that. Do I want to be tied to a chair and made to watch some sadist’s bitchy fantasy, forced agenda, or “sissified?” No. No, I would not. In fact I would probably literally try to murder anyone that attempted it. That’s how viscerally I would take such a thing.

It’s less about being emotionally “attached” so to speak than that the MC is meant to be by the nature of good story telling, relatable and immersive enough to believe in. To root for, as the “hero.” So to speak. You cannot do that if the girl he’s tied to, down whatever romantic rank you want to invent for her arc, is going to cut his balls off. Because the writer has an agenda and does not WANT the character to want to, or be able to fight back. Most people in general will not stand in a relationship and be abused. When they have a choice. In the case of NTR issues, it’s ALWAYS – outside of financial concerns for the male – a clear choice. In reality, they can walk away. Or we would not have the acronym MGTOW. As far as to the games; we can play these games, or we can not play these games. For those that say they would refuse, how can you disrespect that? Could they do it less judgmentally, sure. But no one, not at the core of the matter, likes the idea of being betrayed. Willfully wounded, as is mostly the case in these NTR based games. The question of your definition of “cheating” can be elasticized all day, depending on your innate ability to forgive. For myself, I have played enough of them to know very well they aren’t to my tastes. One thing I can say is they are easier to tolerate when A) It is the MC taking someone else’s wife with the hope to create an actual loving, long-standing relationship in a harem. Because that’s what they were missing from their current one, and B) The player has options to choose how far or whether or not to participate.

The end of it is, NTR writers tend to see it as a power fantasy for them.
A good writer in general, whether NTR based content or not, will understand that it’s bad practice to try to strip power entirely from their audience. No one wants to play at the table of a DM who’s actively aiming for a Total Party Kill. If NTR content gets you off, whoever you are – more power to you. It doesn’t do anything for me in the long term, unless it’s handled with respect to the player. Which has to be done very carefully, and frankly is a VERY rare thing. Most often, it is simply not.

Also, some of us are simply smart enough to know exactly where we stand with the notion we will get anything sexual out of a relationship with a girl. Not every male who chooses not to chase them is automatically an “incel.” I don’t hate them; but I do have trust issues. I am very dominant, and that’s a whole separate thing to consider as far as getting into a sustained relationship. Whether I want one or not, it will also come down to: does she want me. I’m not going to begrude that so far it seems not, and I might be under-selling myself to a point, but I’ve not yet been proven incorrect. I also will not waste time, chasing something that doesn’t want to be chased. It just means I can spend my own money on what I consider fun. Rather than trying to convince someone that for a start, needs to be, to fuck me.
